Docobo, a leading UK telehealth company which enables the remote monitoring of patients with long term health conditions such as asthma, COPD, heart disease and hypertension, is to extend its doc@HOME solution to enable patient interaction via mobile phones. The aim is to provide an unprecedented level of flexibility in disease management for patients through large and small healthcare organisations. It means that patients who lead active lives can monitor their condition whilst out and about.
Docobo’s widely used HealthHUB portable home monitor enables patients to record vital signs, symptoms, lifestyle and quality of life measures around the clock from any location. This information is then securely transferred to the doc@HOME clinical database that can be accessed by clinicians using a standard secure web browser, which provides a presentation of the patient’s health diary and alert indicators. The new mobile platform will allow a larger cohort of patients to monitor aspects of their health through their mobile phone as well as the HealthHUB, depending on their specific needs.
Doc@HOME is a fully versatile system that can be configured to meet the health needs of individual patients; empower patients to participate in their care delivery, support their carers and reduce the burden of care provision through particular information required by clinicians to deliver expert care on a patient by patient basis.

Furthermore there have been reductions in out of hours calls and unnecessary hospital admissions.
